Men's Paid Registration

USA LACROSSE NUMBER

During registration you will be asked to provide a current USA Lacrosse membership number. This is required for insurance purposes. If you do not have a number, or your number has expired, you will be redirected to the USA Lacrosse website prior to registering for the team. Your membership number must be valid through the end of the season (5/30/26) or you will be asked to renew it.

PHYSICAL AND IMPACT TEST

Once you have completed registration, you will receive an email with a link to upload a current physical exam for your player, the exam must be dated no earlier than June 2024. The document must be uploaded prior to the start of the season. If your player has completed a physical for another sport you may request a copy from the school and upload it.

Each player must also have a current impact test. This test will be administered by your school athletic trainer via email. You will be asked for the email you would like the test sent to. Please be on the lookout for it to land in your inbox.

PAYMENT

Club dues are $435 for the season. You can pay it all upfront or opt for a payment plan that splits it into 5 equal payments. If you select the payment plan, there is an initial deposit is $87 with additional monthly payments of $87 withdrawn from your account on the 15th of each month beginning on February 15th.

UNIFORMS

Uniforms are provided by the club. Each player will receive a home and away jersey and shorts, a reversible practice jersey and a team jacket. These items must be cared for and turned back in at the end of the season. Players must purchase their own shooter shirts. These will be yours to keep. If you purchased shooter shirts last year you do not need to purchase new ones. The cost for the shirts is $17 each. You may choose which school logo you would like to appear on your away shooter shirt. All home shooter shirts will have the Nelson logo.

If your player requires a new physical you can download the form below and have it filled out by your physician. The physical exam forms provided at doctor's offices or the school health office are also accepted.
